Abstract

Abstract

Opioids are the mainstay in pain treatment and can be invaluable medications in palliative care. However, they are also responsible for the vast majority of preventable deaths related to drug use. This book presents an overview of the social and pharmacological aspects of opioid effects, aiming to provide researchers and health professionals with a firm understanding of their effects and mechanisms of action. The first section reviews the history of opioids from ancient civilizations to the current opioid crisis, gives voice to opioid users through first-hand testimonies, analyzes the existing international drug treatment agreements, and presents structured interventions for persons who misuse opioids. The second section covers state-of-the-art advances in the medical use of opioids, opioid-induced signal transduction, opioids in pain management, opioid use disorders and their treatment. It also presents an updated view of opioids as regulators of inflammation and host defense against pathogens and damage. The overall aim of this book is to provide the reader with a deeper knowledge of this field despite the complexities associated with the existence of numerous ligands, receptor subtypes, and complex pharmacological profiles. To this end, the chapter authors are specialists in the field who also have extensive experience in teaching, hoping to lend their expertise in translating complex concepts into comprehensive and clear explanations.

Abstract

This book provides the first systematic study on three types of incentives for organ donation. It covers extensive research conducted in four culturally different societies: Hong Kong, mainland China, Iran and the United States, and shows on the basis of the research that a new model of incentives can be constructed to enhance organ donation in contemporary societies. The book focuses on three types of incentives: honorary incentives, commonly adopted in the United States and other Western countries by offering things such as a thank-you card and a memorial park for donors to encourage donations motivated by pure altruism; compensationalist incentives, adopted in the Islamic Republic of Iran to encourage donation by providing monetary compensation to unrelated living donors for appreciating their altruistic contribution of donation; and familist incentives, implemented in Israel and mainland China to provide priority to organ transplantation to donors and/or their family members. The book demonstrates that a new model of incentives must go beyond offering only one type of incentives and should rather include different types of incentives that are practically effective, politically legitimate and ethically justifiable for particular societies. This implies that suitable incentive measures may vary from society to society to optimize organ donation. This book provides a clear reference for both the scholars and practitioners in the field of organ transplantation, as well as for general readers interested in bioethics and health care policy.

Abstract

This book aims to address the infusion sequence of the main protocols used in the treatment of varied solid cancers. Since an inadequate infusion sequence can compromise the patient's treatment, this work will provide support to professionals working in the field of oncology in assessing each chemotherapy infusion sequence. The introductory chapters present the definition, indication, and the risks and benefits of polypharmacy in cancer therapy, and discuss the importance of drug combination in cancer treatment. Chapter 2 focuses on the challenges and also the toxicity of combination therapy in cancer, while chapter 3 highlights the parameters that must be evaluated before defining the infusion sequence, such as pharmacodynamic and pharmacokinetic profiles, drugs' stability when diluted or reconstituted, toxicological profile of each drug, among others. The remaining chapters are divided by type of cancer. The content is focused on solid tumors, dividing the chapters according to breast, gastrointestinal, genitourinary, gynecological, head and neck, lung, and neurological cancers. In each chapter the epidemiological profiles, pathophysiology, therapeutic modalities, and the main chemotherapy protocols are addressed, as well as efficacy studies and data on the infusion sequence of each mentioned protocol. This work will be a valuable resource to physicians, nurses, and pharmacists, and may help to improve health service practices.

Abstract

The current book is focussed on the Toll-like receptors (TLRs), which are the first pattern recognition receptors (PRRs) discovered in humans. For example, TLR4 was first recognized in humans in 1997 as a PRR recognizing the Gram-negative bacterial lipopolysaccharide (LPS). This discovery revolutionized the field of innate immunity and filled the long-standing gap in the pathogen recognition by the immune system. Now, it is well established that humans have 10 (TLR1-TLR10) and mice have 12 (TLR1-TLR13) functional TLRs, excluding TLR10 that is present as a defective pseudogene. TLRs are present as both membrane-bound extracellular (TLR1, TLR2, TLR4, TLR5, TLR6, and TLR10) and intracellular (TLR3, TLR7, TLR8, and TLR9) PRRs in humans, which identify different pathogen or microbe-associated molecular patterns (PAMPs or MAMPs) and death or damage-associated molecular patterns (DAMPs) released by the host cells. A lot of development in the TLR biology has occurred in last 24 years since there first discovery in humans. The book is intended to describe their role in the host defence, human reproduction, non-infectious sterile inflammatory conditions, including brain immunity and cerebrovascular diseases, signaling mechanisms, adaptive immunity, and their targeting for drug development.
